How to Enter
To enter the Aflac Fit Checks Sweepstakes:

Complete the online evaluation.
After completing the evaluation, follow the instructions to checkforcancer.dja.com.
Complete the required entry form, including information such as your name, email address, phone number and preferred jacket size.
Submit the completed entry.
The evaluation does not have right or wrong answers, and your evaluation result does not determine whether you receive an entry. Each eligible person/email address may receive a maximum of one entry during the Sweepstakes Period. Automated entry methods and multiple-entry attempts are prohibited.
Prizes
There are 76 total prize opportunities:
1 First Prize: Aflac Prize Pack consisting of Sponsor-selected Aflac branded items. ARV: $75.
26 Large Jacket Prizes: One Limited Edition Fit Checks Jacket by Rachel Zoe in Large/Extra-Large. ARV: $375 each.
49 Small Jacket Prizes: One Limited Edition Fit Checks Jacket by Rachel Zoe in Small/Medium. ARV: $375 each.
There is a limit of one prize per person/household. No cash alternative or prize substitution is permitted except at the Sponsor's discretion.
Winner Selection
The potential First Prize winner will be selected in a random drawing on or about October 21, 2026. The 26 Large Jacket winners and 49 Small Jacket winners will be selected through separate random drawings based on the jacket size indicated on eligible entries.
The odds of winning depend on the number of eligible entries received. Winners will be notified by email and/or phone.
Who Can Enter?
The sweepstakes is open to legal residents of the 50 United States who are 18 or older as of the date of entry. Employees of Aflac Incorporated, Zeno Group, Don Jagoda Associates and their specified related companies and agencies, along with certain immediate family and household members, are not eligible. The promotion is void where prohibited or restricted by law.
Important Rules
Potential winners may be required to confirm their eligibility and mailing address within five days of the notification attempt. Failure to respond or comply with the Official Rules may result in disqualification and selection of an alternate winner.
The Sweepstakes is governed by the laws of the State of Georgia, except where federal or applicable state law preempts those provisions.
